Self Pay Discount
Patients without health insurance who are paying out-of-pocket receive a 10% discount when services are paid in full at the time of the visit.
Patient Responsibility
Please note that it is the responsibility of the patient or legal guardian to provide our office with complete and accurate insurance information. Patients with more than one insurance plan must notify each insurance carrier of the existence of multiple policies and ensure that their Coordination of Benefits (COB) information is kept up to date annually.
Failure to disclose insurance information on your registration forms, or delays in updating your insurance details, may result in denied insurance claims. In such cases, the patient/guardian will be responsible for all charges. Our office must adhere to insurance company deadlines for claim submission.
Plans We Do Not Participate With
Please verify participation with our staff before your appointment. At this time, our office does not participate with the following plans:
- Martin’s Point & Tricare
- Emblem Health
- Molina
- Wellcare
- AARP
- NY Christian Health
- Today’s Option
- Medicaid (as primary insurance)
- CHP (except Fidelis & BCBS)
- Worker’s Compensation
If your insurance changes to any of the above plans (including as secondary coverage), you must notify our office immediately.
Usual and Customary Rates
Our practice is committed to providing high-quality care and charges fees that are considered usual and customary for this region. Patients are responsible for payment of services regardless of any insurance company's determination of allowable or customary charges.
