**For those patients who do not have health insurance and are self-pay, we offer a 10% discount on services
paid in full at the time of service.
**Please contact the billing department if you have questions if our office participates with your insurance.
PLEASE NOTE: It is the PATIENTS / GUARDIANS RESPONSIBILITY TO NOTIFY AADO OF ALL INSURANCES THAT THEY AND/OR CHILD(REN) ARE ENROLLED IN. PATIENTS WITH MULTIPLE INSURANCES ARE REQUIRED TO NOTIFY EACH INSURANCE COMPANY OF THE EXISTENCE OF MULTIPLE CONCURRENT PLANS, AND ARE RESPONSIBLE TO UPDATE “Coordination of Benefits” annually with each Insurance company. Should you not disclose insurance with whom you have coverage on your Registration Form and your insurance carrier retracts payment, you will be responsible for all charges billed if current information is not provided in a timely manner. Our office must abide by timely submission rules with insurance companies with whom we participate.
Please verify with our office staff if we participate with your insurance. Our office does
NOT participate with Workman's Comp, Medicare, Medicaid, UHC Medicaid government programs, Excellus government products or Total Care. **If your insurance changes to one of the products (including Secondary coverage), you must notify our office immediately.
NY Exchange / Marketplace: See descriptions under Insurance. (Excellus & Fidelis) It is best to check with our billing department before changing to a Marketplace plan to confirm we participate.
Usual and Customary Rates: Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates.
